For the safety of our members, guests, instructors and host facility, all persons attending Balintawak Canada Academy activities must complete the applicable registration and acknowledgement before entering the designated training area or participating in any activity.

By signing this form, you acknowledge that martial arts training involves inherent risks, agree to follow all instructor and facility safety rules, provide emergency authorization where applicable, and accept the terms appropriate to your participation or attendance.

Photo & Video Release

While training and during demonstrations, instructors, parents, visitors and students take photos and video of those participating in Balintawak. These are typically kept in our safe archive and displayed on the Balintawak Canada website, and some are submitted to local newspapers, publications, and used in Balintawak Canada promotional materials.

I hereby release and forever discharge Balintawak Canada and its Instructors, Members and Authorized Guests from any and all actions, claims and demands for damages, loss or injury, however arising, which may hereafter be sustained by me (or by the minor named above) in consequence of attending classes at this school. This includes any claims arising from any disease of the human body. In addition, I agree that neither the instructor nor any of its members shall be responsible for any loss or theft of personal possessions.

Balintawak Canada Group Code of Conduct
  1. Attend all practice sessions regularly and on time as personal schedules permit. Family, work and religious priorities always take precedence.
  2. Use officially approved equipment during practice sessions. A member may be denied participation without it.
  3. Dress to practice and observe proper hygiene (shower fresh, cut your nails, no hat in the dojo, deodorant if you must). No smoking during practice hours.
  4. No foul language, and no alcohol or drugs, during training sessions. Any such person may be asked to leave at the Head Instructor's discretion.
  5. Sit down or stand at ease, keep your stick in its proper handling, and cooperate with and obey all instructions from the instructors respectfully.
  6. Always make the safety and well-being of everyone a top concern and priority. Exercise utmost care to avoid injury to yourself and others.
  7. Respect the law. Avoid anything illegal, immoral, or detrimental to our group and its students. Conviction of a criminal offense is grounds for dismissal.
  8. Be courteous, generous, kind, humble and respectful. Each student deserves the respect, encouragement and support of all the other students.
  9. Be sport and always do your best. Set high personal standards and specific goals, and strive to achieve what our curriculum sets for you.
  10. Balintawak Canada martial arts should only be used for self-defense — not for bragging, clowning, showing off, aggression, or any illegal or immoral activity, or you will be dismissed.
  11. You are encouraged to establish friendships with fellow students. The most noble purpose of our group is to meet together, enjoy our heritage art, and be nourished by the pleasure of becoming someone's friend.

The Balintawak Canada Group reserves the right to modify the Code of Conduct as it sees fit, without prior notice to students.
